After careful consideration, LEAF will revise the pricing structure for programs and services as of March 1, 2024. The decision to revise our fee structure is rooted in our commitment to delivering the highest quality healthcare services. Several factors have contributed to this adjustment including enhanced patient care, rising operational costs and compliance & regulation considerations.
The increased pricing reflects our commitment to providing quality healthcare services that prioritize your needs.

LEAF Weight Management Clinic is located at 1980 Ogilvie Road (Gloucester Mall which is at the corner of Ogilvie and Blair roads). We are inside the Loblaws and the entrance to LEAF is behind Cash 3 of Loblaws. The clinic is on Line 1 of the O-train at Blair station. Parking is free.

A referral to the clinic opens the most options to you. Download our referral form and bring a copy to your doctor or nurse practitioner if they don’t have one. With a referral, you will gain access to working with a LEAF Physician and ability to join the LEAF ROOT or LEAF STREAM program. If you know which program you want to join, then after receiving your referral, please call our office.
If you do not have a referral, then you can access working with a LEAF Registered Dietitian in either the LEAF ACTIONS or the LEAF BUDS programs.

At the present time due to the COVID pandemic, we are primarily providing virtual care but also have limited in-person visits. We will continue to monitor the evolving situation and provide more in-person care when it is safe to do so.
Please note that if you are scheduled for an in-person visit, masks will remain mandatory until further notice.
LEAF STREAM program and LEAF ACTIONS course will remain virtual indefinitely.

LEAF WMC wants to protect the health of our clients and staff. Masks are required for any patient and their family/companion coming in with congestion, cough, or sore throat. If this applies to you, please bring and wear a mask while in the clinic.
For all other patients, masks are optional.

If you are interested in a physician-led program (LEAF ROOT or LEAF STREAM) and have a referral, follow the “Get started” button or call the clinic to let us know so that we can get you started as soon as possible.
If you are interested in a registered dietitian-led service (LEAF ACTIONS or LEAF BUDS), no referral is required to the clinic. Simply follow the “Get started” button for the LEAF ACTIONS or LEAF BUDS program. You also can call the clinic.

Our medical office assistant will arrange with you to book your appointment.
You will receive a reminder for your scheduled appointment twice: 3 days before and the night before your appointment.
If you prefer to schedule an in-person visit, please state this to our medical office assistant. Email reminders will be from an address “noreply@cliniconnex”.
Virtual appointments are by video (ZOOM). If you are scheduled for a video appointment, the night-before appointment reminder will be an email which contains the ZOOM link. Sometimes reminders go into your junk email folder so please look for the email from “noreply@cliniconnex” and mark this as safe.

We offer LEAF Physician and LEAF Registered Dietitian-led programs
LEAF Physician-led programs are LEAF ROOT and LEAF STREAM. To participate in either of these programs you will need a referral from your doctor.
LEAF Registered Dietitian-led programs are LEAF ACTIONS and LEAF BUDS. You do not require a medical referral to participate in these programs.

To remain under the care of your LEAF Physician, you must see them at least once per year. If you do not see your LEAF Physician within 365 days of your last appointment, you will need to have a doctor or nurse practitioner re-refer you to the clinic.

As often as is medically appropriate. This usually means every one to three months, though patients are seen weekly when circumstances warrant this.
Obesity is a chronic medical condition that requires long-term follow-up. And since weight fluctuates, sometimes significantly, we usually ask patients to schedule appointments with their LEAF Physician every three months. This ensures your LEAF Physician can provide timely, effective care and intervene quickly in the event of weight regain.

We take weight regain seriously because it's always easier and better to treat as early as possible. Sometimes, patients who experience weight regain feel embarrassed and don't want to return to the clinic to see their LEAF Physician or LEAF Registered Dietitian until they lose some of the weight they've regained.
Remember, we have helped thousands of people manage their weight. We understand that your weight journey is a marathon, not a sprint. We will never judge or shame you for regaining weight.

No.
Though we are a program-based clinic, you are not required, nor will we pressure you to enrol in a program. You can schedule appointments with your LEAF Physician as often as you need, subject only to medical appropriateness and appointment availability.

While none of our programs (LEAF ROOT or LEAF STREAM, LEAF ACTIONS or LEAF BUDS) are covered by OHIP, some insurance plans do cover the cost of the program component but not the cost of meal replacements or food. Please contact your insurance to see if you have coverage for lifestyle or health programs or for registered dietitian services.
LEAF Physician consultation and follow-up visits are OHIP-covered provided you have a valid OHIP card.

You can pay by credit card. We do not charge a convenience fee if you choose to pay by credit card. We do not accept cash, e-transfer, or cheque.
Programs must be paid in full and will not start until payment has been received.

By joining a LEAF program (LEAF ROOT or LEAF STREAM), you are committing to working with the LEAF team for a 24-week period. However, we understand that circumstances do arise. We have limited flexibility to extend your program. As such, all sessions must be completed within 32 weeks from the date of your first program appointment or they expire.

When enroling in the LEAF ACTIONS course, you will have access to the course material for 24 weeks. The LEAF ACTIONS course is a closed group course and therefore no extensions are available.
LEAF BUDS is pay as you go.

If you decide to withdraw from a LEAF program; LEAF ROOT, LEAF STREAM or LEAF ACTIONS before initiation of the first session, then you will receive a full refund minus an administrative fee of 5.0%.
If within 2 weeks of a LEAF program; LEAF ROOT, LEAF STREAM or LEAF ACTIONS initiation, you decide to withdraw, you will receive a refund minus 25% of the initial fee. After 2 weeks, no refund will be provided.
LEAF BUDS is non-refundable.

We require a minimum of 2 business days notice to cancel or rebook an appointment with either the LEAF Physician or LEAF Registered Dietitian.
An appointment with the LEAF Registered Dietitian is considered to have been used if you have provided less than 2 business days to reschedule or no showed to your appointment. No refund will be provided for that appointment. In the case of a program, you have used one of your sessions unless you choose to pay a no-show fee.
An appointment with the LEAF Physician that is cancelled with less than 2 business days notice or no show on the date of your appointment may be subject to a no-show fee.
The no show fee for both physician and dietitian are as follows:
$100 for a consultation/first appointment.
$45 for a physician follow-up
$60 for a dietitian follow-up
Due to high volume, patients who no show to their consultation appointment with a LEAF Physician will have their referral returned to their provider. If you have missed your LEAF Physician consultation appointment and remain interested in having a consultation, then please have your doctor or nurse practitioner re-refer you.
Patients who no show on two consecutive follow-up visits with the LEAF Physician will be discharged from the clinic.

In general, LEAF Physicians and LEAF Registered Dietitians provide services mainly to people living in Ontario. The following exceptions apply to people living in provinces outside of Ontario.
From Newfoundland, Nova Scotia & New Brunswick:
People living in these provinces may sign up for LEAF Registered Dietitian services which include LEAF ACTIONS and LEAF BUDS.

Optifast®900 is a complete meal replacement which means that one Optifast®900 shake has ¼ of your daily allowance of macro and micronutrients. Each shake is 225 calories and costs about $3.50/per shake.
In the LEAF ROOT program, there is NO obligation to use Optifast®900. With the guidance of the LEAF Registered Dietitian, you may choose to either be completely food focussed with no meal replacements or you might want to try replacing 1 or 2 meals in a day with Optifast®900. Your LEAF Registered Dietitian will help you with your nutrition prescription.
In the LEAF STREAM program, Optifast®900 MUST be used in the program as a full meal replacement strategy (which means having 4 Optifast®900 shakes a day).
Optifast®900 may only be purchased by prescription from a LEAF Physician from the Loblaws Pharmacy at 1980 Ogilvie Road. Optifast®900 is not eligible for reimbursement by private medical insurance plans as it does not have a DIN number.

If your prescription for a medication or Optifast®900 has run out, then you must book an appointment with your LEAF Physician to obtain a renewal. LEAF Physician’s need to see you in follow-up to see how you are responding to treatment and decide if renewal of your prescription is medically appropriate.

Yes... lots... and it's free!
Our clinic is located inside the Loblaws at the Gloucester Shopping Center. There literally are hundreds of parking spaces right out front of our clinic... all free.
