Is ROOT™ Right for You?

ROOT™ might be right for you if your goal is to lose 5% to 10% of your starting weight with the aid of a partial meal replacement (up to 2 shakes per day).

ROOT™ Plus

Want to add more structure to the program and more support from your registered dietitian?  Enrol in ROOT™ Plus for an additional $150.  ROOT™ Plus adds a personalized meal plan created by your registered dietitian and smart shopping lists that you can access through our online meal planning portal. You also get two check-in calls from your registered dietitian to make sure you are getting the full benefit of this amazing add-on.

Additional Check-Ins

Want a little more support and guidance from your registered dietitian?  You can further customize your program by adding up to two additional check-ins between each appointment. Add:

  • one check-in between appointments (total of 5) for $150; or
  • two check-ins between appointments (total of 10) for $300.

Convenient

Life is hectic. We get it! That is why we offer virtual appointments for those who cannot or do not want to drive to the clinic. Meet with our doctors by video or phone. Our video appointment software allows you to communicate face-to-face with your doctor and dietitian over an encrypted channel.

How ROOT™ and ROOT™ Plus Work

You will receive coordinated care from one of our registered dietitians and one of our doctors who specializes in bariatric care.  This means you will see both a registered dietitian and doctor at each of the six program appointments.

Consult

Your first appointment is a nutrition consult with your registered dietitian. The appointment lasts 50-60 minutes.  During this appointment, your registered dietitian will learn about you, your weight journey, your health, and your nutrition, including your food taste preferences, hunger patterns, and what has worked and not worked for you regarding your weight management.
Your registered dietitian will help you set healthy, achievable goals and provide you with nutrition counselling.  They will create a nutrition and behavior change plan for you to help you meet your nutrition needs and reach your health and weight goals.
Near the end of the appointment, your registered dietitian will invite your LEAF doctor to join in. Your doctor will be updated on your goals and your nutrition plan. They also will ask you questions to ensure proper medical oversight.
If you enrolled in ROOT™ Plus, after the appointment, your registered dietitian will create a personalized meal plan for you, which you will access online through our amazing meal planning portal that includes smart shopping lists.

Follow-Ups

Your second through sixth program appointments with your registered dietitian will last 20-30 minutes. During these appointments, your registered dietitian will ask you about your progress, provide nutrition counselling, adjust your nutrition plan if necessary, and keep you moving forward toward your health and weight goals.
Near the end of each appointment, your registered dietitian will invite your LEAF doctor to join in. Your doctor will be updated on your progress and may recommend additional therapeutic options as they deem appropriate. They also will continue to ask you questions to ensure proper medical oversight.

After the Program

Obesity is a chronic disease that requires long-term management. After you complete your program, we offer regular appointments with your LEAF doctor. Regular appointments with your LEAF doctor (OHIP covered) will ensure we catch weight regain as soon as it occurs and we deal with it before it is significant. Our standard post-program follow-up schedule will help you be successful over the long-term.
Some patients who complete a program decide to re-enroll in the same program, a different program, or a membership. Others choose the less structured approach of BUDS appointments. Whatever you choose, we will always be here for you and we will never pressure you to enroll in any of our programs or memberships. At LEAF, you are in control of your care.

Ready to Get Started?

All programs require medical authorization. If you already are a patient at LEAF and were referred to the clinic by a doctor or nurse practitioner, call or email the clinic to schedule an appointment with your LEAF doctor to let them know you want to enroll in ROOT™. Otherwise, have your doctor or nurse practitioner refer you to LEAF for a medical consult.  We usually see patients in consult within two to three weeks of referral.
